Send your short CV, choice of course & month to find your eligibility and availibility of course for initial enrollment.
If you are eligible and seats are available, we shall ask you to send the following documents:
1. Copy of your highest academic qualification.
2. One passport sized photograph.
3. Residence proof (copy of passport is the best).
4. One recommendation letter in their letterheads from any of the two of the following persons:
a) Head of his/her Institution.
b) Principal, Superintendent or Head of the departments of any Medical college.
c) Secretary or President of State or National branch of ISSP, ISA, IOA, NSI, IASP, WIP or similar association/societies.
This recommendation letters will be addressed to "The Director, Daradia: The Pain Clinic". It must contain the present working place.
Send all these documents by email attachment for initial enrollment.
Payment of non-refundable & non-transferable registration fee (Rs. 4000.00 for Indian and USD 250.00 for International doctors) confirms registration & should be paid by demand draft or cash (or bank transfer for International participants only) during enrollment. Remaining part of course fee should be paid before due date i.e. one month before starting of course. Registration may be cancelled and other candidate may be taken if full course fee is not paid before due date.
